Prostate cancer final stages: what to know

Cancer may have spread to nearby tissues, lymph nodes, bones, or other organs. The stage is assessed with scans, blood tests, examination, and sometimes tissue testing. Symptoms vary: some people have pain or tiredness, while others have few symptoms. Bone involvement can weaken bones and affect movement. Urinary changes may also occur; difficulty passing urine deserves medical assessment, but it does not by itself show that cancer is advanced.

The outlook depends on where the cancer has spread, how quickly it is changing, previous treatment, general health, and response to the next plan. A clinician can explain what the findings mean for one person.

Treatment may aim to slow cancer, reduce symptoms, protect bones, or support daily function. Options can include hormone-based treatment, chemotherapy, radiotherapy, targeted radioisotope therapy, and bone-strengthening therapy. Procedures for urinary blockage or pain may be considered. Palliative care can be added at any stage to manage pain, nausea, breathlessness, sleep problems, and emotional strain; it does not mean that cancer treatment has stopped.

Bring a current medicine list and note pain, urinary changes, appetite, sleep, and mobility. Ask who to call outside appointments and whether a written symptom plan is available. Support from family, counselling, nutrition, physiotherapy, or spiritual care can be part of treatment.

Contact the oncology team promptly for new or worsening severe pain, inability to pass urine, repeated vomiting, confusion, fever, or a sudden decline in strength. Seek emergency care for new leg weakness, numbness around the groin, or loss of bladder or bowel control, because pressure on the spinal cord can require urgent treatment. Do not wait for a routine visit when these changes appear.
