What shoulder tenderness may indicate

Pain at the front, side, or top of the shoulder can arise from several structures. Rotator Cuff Tendinitis, Shoulder Impingement, Calcific Tendinitis, and a Rotator Cuff Tear may affect lifting or reaching. Frozen Shoulder often causes marked stiffness, while Shoulder Osteoarthritis can cause aching and reduced movement. Neck nerve irritation, called Cervical Radiculopathy, may send pain or tingling toward the arm.

The spot that hurts when touched is not always the structure causing the problem. Shoulder Pain is best understood alongside movement limits, weakness, injury history, and symptoms in the neck or arm.

A cautious approach

If there is no recent major injury or warning symptom, use light pressure on the surrounding muscle rather than digging into the joint, collarbone, or front of the neck. Keep the shoulder moving gently within comfort and reduce repeated overhead work for a while. Stop if pressure causes sharp pain, numbness, weakness, or a lasting flare.

Do not force a stiff shoulder or repeatedly test painful lifting. A clinician or physiotherapist can assess strength and range of motion and guide suitable exercises. A sudden loss of function needs assessment rather than self-massage.

When to seek medical care

Seek urgent help for shoulder pain with chest pressure, breathlessness, faintness, or sweating. Prompt assessment is needed after a fall or collision, visible deformity, sudden weakness, inability to raise the arm, fever, marked redness, or a hot swollen joint. Arrange review when pain persists, disturbs sleep, or includes progressive arm numbness.
