Pregnant spotting after a bowel movement

Bright blood on toilet paper after straining, with anal soreness or itching, can be consistent with bleeding from the rectum. Blood on underwear or a pad, blood mixed with vaginal mucus, or bleeding that continues after you have finished passing stool suggests a vaginal source instead.

Vaginal spotting has several possible explanations, including cervical irritation, infection, or changes in the cervix. A symptom does not identify one condition by itself. A clinician may consider causes such as [Chlamydia](/conditions/chlamydia/), [Cervical Dysplasia](/conditions/cervical-dysplasia/), [Endometrial Polyps](/conditions/endometrial-polyps/), or [Uterine Fibroids](/conditions/uterine-fibroids/) when appropriate.

Use a clean pad to observe the colour and amount, and record when it began, whether it followed straining, and whether pain or discharge accompanied it. Avoid inserting anything into the vagina until you have received advice. Contact your maternity team or clinician to report any vaginal bleeding, even when it is light.

If the bleeding is clearly from the rectum, mention it at your next review or sooner if it recurs, becomes painful, or is accompanied by black stool.

Seek urgent assessment for heavy or increasing vaginal bleeding, severe abdominal pain, one-sided pain, fainting, shoulder-tip pain, fever, or feeling very unwell. During pregnancy, these warning signs need prompt evaluation rather than observation at home.
