Pregnancy sickness with blood in vomit: what to know

After forceful or repeated vomiting, small streaks of fresh blood can come from irritation or a small tear in the throat or food pipe. Dark brown material that resembles coffee grounds can indicate older blood. Other causes include inflammation, an ulcer, or bleeding higher in the digestive tract. Pregnancy sickness can trigger vomiting, but it does not explain away blood.

Blood in vomit is a symptom, not a diagnosis. Rarely, digestive cancers such as oesophageal cancer or stomach cancer can cause bleeding, but many non-cancer causes are more likely. Assessment may include questions about the colour and amount, medicines, pain, bowel motions, and pregnancy symptoms.

Contact your maternity team, urgent care service, or emergency department promptly for advice. Do not drive yourself if you feel faint. Keep a note of when the vomiting began, what the blood looked like, how often it happened, and any medicines you take. If you can swallow safely, take small sips of fluid while arranging care; do not force food or drink if vomiting continues.

Avoid aspirin, ibuprofen, or other medicines that have not been approved for you in pregnancy unless a clinician specifically directs them. Bring your pregnancy records and a list of medicines. A clinician can check hydration, pulse, blood pressure, blood count, and the digestive tract while considering the pregnancy.

Call emergency services or go to an emergency department immediately if the bleeding is heavy or ongoing, the vomit looks like coffee grounds, or you have faintness, confusion, severe weakness, fast breathing, or a racing heartbeat. Severe abdominal or chest pain, black stools, inability to keep fluids down, or very little urine also require urgent help.

Tell the maternity team urgently about vaginal bleeding, severe pelvic pain, or reduced fetal movement when relevant to your stage of pregnancy. These warning signs do not prove a cancer diagnosis; they signal that you may need rapid assessment and treatment.

Ask: Where might the blood be coming from? Do I need blood tests, fluids, or a digestive examination? Which anti-sickness medicines are suitable during pregnancy? How should I take my usual medicines? What symptoms mean I should return immediately? When should my maternity team review the pregnancy?
