What should you know about posterior shin splints running?

Short answer

Posterior shin splints are exercise-related pain along the inner or back edge of the shin, often after a training change, and they improve with sensible load adjustment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How running can stress the shin

Repeated impact and calf work can irritate tissues where they attach along the inner border of the shin. Runners may notice a dull ache during a session, tenderness along a broader strip, or stiffness after activity. A sudden increase in distance, speed, hills, or running days can outpace recovery. Flat feet or an inward-rolling gait may alter how load travels through the ankle and lower leg, but these features do not by themselves prove the cause. Pain that becomes very localized, worsens with walking, or appears at rest needs a different assessment because a bone stress injury can resemble shin splints.

Adjusting training

Temporarily replace painful runs with cycling, swimming, or another comfortable option. Reduce hills, speed work, and hard surfaces when you restart. Check that shoes are in suitable condition, and avoid trying to run through a steadily worsening ache. Strength work for the calf, foot, and hip can support a gradual return when it is comfortable. Physiotherapy may include a running assessment and gait retraining; custom orthotics may be considered when foot mechanics and symptoms point in that direction. Increase one training variable at a time and allow recovery between harder sessions.

When to seek care

Book an assessment if symptoms keep returning, interfere with walking, or fail to improve after reducing running load. Seek prompt care for pinpoint bone tenderness, pain at rest or overnight, visible swelling, or inability to hop or bear weight comfortably. Urgent help is needed for severe escalating lower-leg pain with tightness, numbness, weakness, or a foot that becomes unusually pale or cold. These signs are not typical training soreness and should not be managed by simply changing shoes.

Questions for your doctor

Ask whether the pain pattern fits shin splints or needs investigation for a stress injury. Ask how much running is safe now, which cross-training is suitable, and what return-to-running stages to follow. You can also ask whether foot posture, footwear, strength, or running form should be assessed.

Questions people ask

Where are posterior shin splints felt?

They are often felt along the inner or back edge of the lower leg, with tenderness spread over a broader area rather than one exact bony point.

Can I prevent shin splints from returning?

Gradual training changes, recovery days, suitable footwear, strength work, and correction of contributing running mechanics may reduce recurrence.
