Pneumonia Not Getting Better With Antibiotics

Pneumonia that is not improving with antibiotics needs medical review to check the diagnosis, treatment response, and for complications. Cough, tiredness, chest discomfort, and breathlessness can take time to settle, but worsening symptoms or a lack of expected improvement should not be managed by changing or adding antibiotics yourself.

Persistent symptoms can have several explanations. The infection may be caused by an organism that the prescribed medicine does not target, or the illness may not be bacterial. The diagnosis may need reconsideration because other chest conditions, including acute bronchitis, can cause a continuing cough. A clinician may also check whether doses were missed, vomiting prevented absorption, or another medicine is affecting treatment.

A review may include listening to your lungs, checking oxygen levels and vital signs, and deciding whether blood tests or chest imaging are needed. More serious infection, fluid around the lung, or another complication can require hospital assessment.

Contact the prescriber or a healthcare service promptly if you are not improving as expected. Tell them when treatment started, the antibiotic name and dose, how often you take it, and whether any doses were missed or brought back up. Describe your temperature, breathing, chest pain, cough, sputum, energy level, and ability to drink and eat.

Take the medicine exactly as prescribed unless a clinician tells you to stop. Do not use leftover antibiotics or share medicine. Rest, drink fluids if you can do so safely, avoid smoking, and use only symptom medicines that are suitable for you.

Seek emergency help for severe difficulty breathing, blue or grey lips, new confusion, fainting, severe chest pain, or inability to stay awake. Get urgent medical advice for breathing that is getting worse, coughing blood, repeated vomiting, inability to keep fluids down, or a marked deterioration after starting treatment. People who are older, pregnant, immunocompromised, or living with significant heart or lung disease may need a lower threshold for review.
