Plaster of Paris hardware: what to know

A Plaster of Paris cast is not only the hardened plaster layer. Soft padding protects the skin and creates space around bony areas. A stockinette or similar underlayer can make the edges more comfortable. Bandages and reinforcement help hold the moulded shape while the material sets. The clinician selects and positions these parts according to the body area, swelling, and support needed.

These materials should not be adjusted at home. A cast that becomes loose may no longer support the injured area, while a cast that becomes tight can put pressure on skin, nerves, or circulation. Similar casting methods may be used in fracture care and staged orthopaedic casting, including clubfoot casting.

Keep every layer dry and intact. Do not pull out padding, cut the edge, add household materials, or insert anything to relieve itching. Keep the limb raised when swelling is present, move the exposed digits as instructed, and follow limits on walking, lifting, and impact. Protect the cast from heat and knocks. If a strap, padding edge, or outer layer rubs the skin, contact the care team rather than repairing it yourself.

Get prompt medical advice for increasing pain, pressure, numbness, tingling, colour change, coldness, or reduced movement in exposed fingers or toes. Report a cast that is wet, cracked, soft, loose, tight, or foul-smelling, as well as drainage, fever, or skin pain. Sudden swelling or a new rubbing point may mean the fit needs checking.

Ask what each layer does, whether the cast can be exposed to water, how to protect the edges during sleep, and which changes require an early appointment. Ask whether the cast will be changed as swelling settles and who should handle removal.
