Placental abruption is premature separation of a normally attached placenta. It often causes abdominal or back pain, contractions, tenderness, and bleeding that may be concealed. Placenta previa means the placenta lies low in the womb and may cover or approach the cervix, the opening to the birth canal. It commonly presents with painless vaginal bleeding, though symptoms vary. Ultrasound and examination by a maternity team help distinguish these conditions. A person can have bleeding for other reasons as well, so symptoms alone cannot establish the diagnosis.

What to do about bleeding

Contact the maternity unit immediately for bleeding during pregnancy, especially with pain, contractions, weakness, or reduced fetal movement. Use emergency services for heavy bleeding, faintness, severe pain, or feeling unwell. Avoid inserting anything into the vagina until a clinician advises otherwise when placenta previa is possible. The care plan depends on bleeding, pregnancy stage, placental position, and the wellbeing of both patients. Birth may be planned or urgent, sometimes by Caesarean Section.

When urgent care is needed

Do not wait for bleeding to stop if it is new, recurrent, or accompanied by pain or a firm abdomen. Reduced fetal movement, dizziness, pallor, breathlessness, or collapse requires emergency help. Placenta previa bleeding can become sudden, while abruption can cause serious concealed bleeding, so the amount seen externally is not a reliable guide to urgency.
