What does placental abruption in the second trimester mean?

The second trimester is a period when the placenta is still supporting a developing baby and birth may not yet be an immediate option. Symptoms can include vaginal bleeding, pelvic or abdominal pain, backache, uterine tenderness, or contractions. Some people have little visible bleeding because blood remains behind the placenta. The outlook and treatment depend on the amount of separation, bleeding, gestational stage, fetal wellbeing, and whether the situation is stable.

Contact your maternity service urgently for bleeding, pain, contractions, or a sudden change in how you feel. At hospital, clinicians may examine you, check vital signs, monitor the pregnancy, perform blood tests, and use ultrasound. If you are stable, observation and follow-up may be possible; if bleeding or fetal concerns become severe, hospital treatment and delivery planning are needed. A Caesarean Section may be discussed when rapid birth is medically necessary.

Call emergency services for heavy bleeding, severe or persistent pain, fainting, marked weakness, trouble breathing, a firm or tender abdomen, or fluid loss. Reduced fetal movement later in pregnancy also requires urgent review. Do not wait for bleeding to become obvious: concealed bleeding can still cause serious illness.

Ask where the placenta is located, what the scan and blood tests show, how the pregnancy will be monitored, which symptoms require return, and how treatment decisions may change if bleeding continues.
