Pins and needles from shoulder to hand: what it can mean

A sleeping position, leaning on an elbow, tight clothing, or prolonged desk posture may briefly compress a nerve. Neck muscle spasm or a problem involving the nerves leaving the spine can send tingling down the whole arm. Recurrent or spreading symptoms may also occur with peripheral neuropathy, Type 2 Diabetes, Vitamin B12 Deficiency, or Hypothyroidism. Anxiety or rapid breathing during Panic Disorder can cause tingling, often around the mouth or in several limbs, but the pattern still deserves attention if it is new.

Change position, gently move the neck, shoulder, wrist, and fingers, and avoid resting your weight on the arm. Note when the sensation starts, how long it lasts, which fingers are affected, and whether pain, weakness, or neck movement changes it. Arrange a primary-care assessment if it keeps returning, lasts beyond a brief episode, disturbs sleep, or affects grip.

Seek emergency help immediately for sudden one-sided numbness with facial drooping, trouble speaking, new vision trouble, severe dizziness, or loss of balance; these can be signs of Stroke or a Transient Ischaemic Attack. Get urgent assessment for new arm weakness, loss of bladder or bowel control, severe neck pain after an injury, or a cold, pale, or blue hand.
