What should you know about PI-RADS 5 prostate cancer?

PI-RADS is a structured way to describe prostate MRI findings. Category 5 means the scan shows a very suspicious area for clinically important prostate cancer. The score describes the appearance on imaging; it does not identify the exact cell type, grade or stage. Benign prostate changes can sometimes look concerning, so the report must be considered with the PSA result, examination, symptoms, previous biopsies and personal history.

A radiologist may describe the location, size and relation to the prostate capsule or nearby structures. These details help the urology team decide how to investigate the finding and whether a targeted biopsy is appropriate.

Arrange review with a urologist or prostate cancer team and bring the MRI report, images if available, PSA history and medication list. Ask whether targeted and systematic biopsy are recommended, how the biopsy will be performed, and how infection prevention is handled. If cancer is confirmed, further assessment guides choices such as surveillance, surgery, radiotherapy or other treatment. Difficulty passing urine should also be reported because it may need separate assessment.

Seek prompt medical help for inability to pass urine, fever or chills after a biopsy, heavy bleeding, severe pelvic pain, or feeling faint. Emergency care is needed for severe weakness, confusion or breathing difficulty.
