What should you know about phenytoin skin rash?

A phenytoin skin rash can be flat, bumpy, itchy, hive-like, or widespread. It may begin after the medicine is started or after a dose change. The rash itself does not show how severe the reaction will become, so a clinician will ask about timing and inspect the skin rather than relying on a photograph alone.

A Drug Rash can affect skin only, but concerning reactions may also cause fever, facial swelling, skin pain, blisters, peeling, or sores on moist surfaces such as the mouth and eyes. Tell the clinician about every medicine and supplement, including anything started recently, because more than one product may irritate or sensitise the skin.

Call the clinician who prescribed phenytoin or seek same-day medical advice when a new rash appears. Keep taking, stopping, or replacing the medicine only according to an individual medical plan; an abrupt unsupervised change may increase seizure risk. Bring the medication container and note the first day of the rash, its spread, and any new symptoms.

Use lukewarm water and a plain moisturiser if the skin is dry, and avoid scratching, sun exposure, fragranced products, and unrecommended medicated creams. Do not cover a rapidly changing rash with cosmetics, since this can make assessment harder.

Go to emergency care for breathing difficulty, swelling of the lips or tongue, collapse, extensive blistering, peeling, severe skin tenderness, fever with rapid spread, or sores involving the mouth, eyes, or genitals. These features require urgent evaluation even if the rash began mildly.
