What should you know about phenytoin rash treatment?

A drug rash may appear as red or itchy patches, raised spots, hives, or widespread peeling. The timing after starting phenytoin or changing its dose, the speed of spread, and symptoms elsewhere help a clinician judge the reaction. A Drug Rash may resemble other skin problems, so do not assume that a new eruption is eczema, infection, or an allergy to a different product.

Some serious reactions can involve the mouth, eyes, or genitals, or cause fever and marked illness. A rash can also occur without severe symptoms at first. Keep the medicine packaging and a list of every prescription, supplement, and over-the-counter product available for the assessment.

Contact the prescriber or an urgent medical service as soon as you notice a new rash while taking phenytoin. Do not restart a stopped dose, change the dose, or substitute another seizure medicine without a clinician’s plan; sudden changes can allow seizures to return. A clinician may decide whether phenytoin should be withheld and what alternative seizure protection is appropriate.

Until you are assessed, avoid new creams, exfoliants, and fragranced products on the affected skin. Take clear photographs in good light, note when the rash began, and record fever, pain, swelling, blisters, or skin tenderness. Do not take someone else’s steroid or antibiotic medicine.

Seek emergency care immediately for trouble breathing, facial or tongue swelling, faintness, blistering, skin that is peeling or very painful, sores in the mouth or eyes, fever with a rapidly spreading rash, or difficulty swallowing. These warning signs can indicate a severe medication reaction and should not wait for a routine appointment.
