Period-like bleeding at four weeks pregnant

Light spotting can occur in early pregnancy, but bleeding that resembles a period, contains clots, or comes with cramps should not be assessed by appearance alone. The cervix can bleed more easily in pregnancy, and some pregnancies continue normally after early bleeding. Other causes include pregnancy loss, infection, or an ectopic pregnancy, where the pregnancy develops outside the womb.

Symptoms can change quickly, and a home test cannot show where a pregnancy is located. A clinician may use your symptoms, examination, pregnancy hormone testing, and an ultrasound when the timing makes it useful.

Contact your maternity service, early-pregnancy unit, or clinician promptly and describe the colour, amount, clots, pain, and when the bleeding began. Use a pad so you can observe the flow; avoid tampons while bleeding. Rest if needed, keep hydrated, and ask which pain relief is appropriate in pregnancy. Do not wait for a routine appointment if symptoms are increasing.

Go to emergency care for severe or one-sided lower abdominal pain, shoulder-tip pain, dizziness, fainting, marked weakness, or heavy bleeding. Urgent assessment is also needed if you feel suddenly unwell, even if the bleeding seems light. These symptoms can occur with ectopic pregnancy, which can become dangerous if it causes internal bleeding.
