Perineal tear not from childbirth: what to know

Short answer
A perineal tear not from childbirth can result from friction, sexual activity, accidental injury, or skin disease and should be assessed if it is deep, painful, or slow to improve.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How it may happen
The perineum is sensitive skin and soft tissue between the vaginal opening and anus. A tear may follow stretching or friction during sex, a fall or impact, shaving-related trauma, or inflammation that makes skin fragile. The appearance alone cannot show how deep the injury is. A clinician may check the wound, nearby structures, infection, and any condition affecting the skin.
First steps at home
Rinse gently with lukewarm water, pat dry, and avoid scented products or rubbing. Pause sex, tampons, and activities that pull on the area until it has healed or a clinician advises otherwise. Wear breathable underwear and use simple pain relief only as directed for you. Do not apply antiseptics, creams, or household remedies inside the wound. Perineal Repair may be relevant when a clinician finds a wound needing closure; Postnatal Physiotherapy is mainly relevant if pelvic-floor symptoms persist after assessment.
Warning signs
Arrange medical assessment for a deep or gaping wound, ongoing bleeding, severe pain, repeated tearing, or symptoms that are not improving. Seek urgent care for heavy bleeding, faintness, fever, rapidly increasing swelling, pus, or difficulty passing urine. A tear after an assault needs prompt medical and emotional support; if you are in immediate danger, contact emergency services and move to a safe place.
What to ask
Ask what may have caused the tear, whether it needs closure, how to clean it, and which activities to avoid. Ask how infection or a skin condition will be checked, what pain relief is suitable, and exactly when you should return if the wound changes.

Questions people ask
Can sex cause a perineal tear?
Yes. Friction or stretching can injure the tissue, especially when it is dry, irritated, or already fragile.
Should a small tear be checked?
A minor superficial tear may settle, but assessment is sensible if bleeding continues, pain is significant, or healing does not progress.
