Perineal episiotomy

A clinician may recommend an episiotomy when there is a concern about the baby’s wellbeing, when an assisted birth needs more room, or when a tear appears likely to extend in a way that could cause greater injury. It is not needed for every vaginal birth. The cut is usually made after local anaesthetic or another form of pain relief, unless birth circumstances require immediate action.

Keep the area clean with plain water, pat dry rather than rubbing, and change sanitary pads regularly. Wash your hands before and after care. Rest when possible, follow prescribed pain-relief instructions, and prevent constipation with fluids, fibre, and any recommended stool softener. Avoid inserting anything into the vagina until a clinician says healing is adequate. Perineal Repair may be discussed if healing needs further attention.

Contact your maternity team if pain is worsening, the wound opens, swelling increases, discharge smells unpleasant, or the area becomes increasingly red or hot. Seek urgent help for heavy bleeding, fever with feeling unwell, severe pain, or difficulty passing urine.
