What can happen
Inflammation may produce pain, itching, swelling, skin tags, cracks, ulcers or pus-like drainage. A fistula is an abnormal channel between the bowel and nearby skin; an abscess is a trapped collection of infection that can cause a tender lump and fever. Bowel symptoms such as Diarrhoea or Abdominal Pain may occur, although local symptoms can need attention even when bowel symptoms are mild. The pattern does not confirm Crohn's disease by itself, because infections and other skin conditions can look similar.
Assessment and care
Arrange a GP Consultation or gastroenterology review for ongoing pain, drainage, bleeding or a new lump. Assessment may include an examination, blood tests, stool tests, imaging or an examination under anaesthesia, depending on the findings. Treatment can combine medicine to control bowel inflammation with antibiotics, drainage of an abscess, or surgery for a fistula. Keep the area gently clean, avoid squeezing a lump, and tell the clinician about allergies, medicines and previous procedures.
When to seek urgent care
Seek same-day care for severe or rapidly increasing pain, fever, chills, a hot swollen lump, worsening redness, inability to pass stool or urine, or feeling faint. An abscess may need prompt drainage and should not be treated by squeezing it. Heavy bleeding, collapse or severe weakness needs emergency help. Contact your treatment team quickly if drainage stops while swelling and pain increase.
