Pelvic pain during the fertile window

Ovulation-related discomfort is often felt on one side of the lower abdomen and may settle on its own. Pain can also come from conditions affecting the uterus, ovaries, or pelvic tissues, including endometriosis, ovarian cysts, adenomyosis, fibroids, or pelvic inflammatory disease. The timing alone cannot identify the cause.

If pregnancy is possible, one-sided pain with bleeding or faintness requires prompt assessment because ectopic pregnancy can present with pelvic pain.

Note when the pain starts, which side it affects, how long it lasts, and whether bleeding, discharge, fever, nausea, or urinary symptoms occur. Rest, gentle heat, and usual pain relief that is suitable for you may help. Avoid activities that clearly worsen the discomfort. A clinician can examine you and decide whether testing or an ultrasound is appropriate.

Seek urgent care for sudden intense pain, fainting, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, or pain during a possible pregnancy.
