What nappy rash can look like

The rash may be red, sore or chafed on the buttocks, genitals, lower abdomen or upper thighs. Wetness, rubbing, infrequent changes, diarrhoea or a new product can weaken the skin barrier. A yeast rash may be bright red, involve the skin folds and have small spots around its edge. That pattern needs different treatment from simple irritation, so avoid assuming every nappy rash needs the same cream.

Practical care

Change nappies promptly, rinse with lukewarm water or use fragrance-free wipes, and pat rather than rub. Give the area brief nappy-free time when practical, and apply a thin protective layer to clean, dry skin. Avoid fragranced products, talcum powder and vigorous scrubbing. Emollient Therapy may help support the skin barrier. If a clinician identifies yeast, they may recommend Topical Antifungal Treatment; do not use a medicated product without advice about the baby's age and the affected area.

When to seek care

Contact a clinician if the rash is severe, blistered, weeping, bleeding, crusted, spreading beyond the nappy area or not improving with careful routine care. Seek prompt help if the baby has fever, seems unusually sleepy, feeds poorly or appears very unwell. A rash that repeatedly returns may need a review of products, bowel symptoms or another skin condition.
