Painful swelling in the feet during pregnancy

Pregnancy can increase fluid retention and pressure in the veins returning blood from the legs, causing swelling that is worse later in the day. Aching can follow stretched skin, footwear pressure or prolonged standing. Swelling that appears suddenly, affects the face or hands, or comes with headache or vision changes can be a sign of pre-eclampsia and should not be dismissed as ordinary oedema.

Rest with your feet raised, change position regularly, avoid standing for long periods, and wear comfortable supportive shoes. Gentle walking and ankle movements can help circulation. Do not restrict fluids or start medicines without advice. Mention painful swelling at your antenatal appointment so blood pressure and urine can be checked, especially if the symptom is new or increasing.

Contact your maternity unit promptly for sudden swelling, swelling of the face or hands, a severe headache, blurred vision or flashing lights, upper abdominal pain, breathlessness, or feeling very unwell. One leg that is much more swollen, red, warm or painful also needs urgent assessment. Call emergency services for severe breathlessness, chest pain or collapse.

Ask whether your blood pressure and urine need checking, whether the swelling is even on both sides, which pain-relief measures are suitable, and which symptoms should lead you to contact the maternity unit again.
