Painful Rash on the Hands and Feet: What to Know

A painful rash on the hands and feet needs attention to its pattern, exposures, associated symptoms, and speed of change. Cracking, blisters, scaling, swelling, or tenderness can occur with several different conditions, so appearance alone is not enough to choose treatment.

Contact dermatitis may follow soaps, gloves, cleaning products, metals, or other exposures. Atopic dermatitis can cause dry, itchy, inflamed skin with cracks. Psoriasis may create sharply defined, scaly areas, while ringworm can form an expanding scaly edge. Scabies often causes intense itch and may affect household contacts. Heat rash, intertrigo, or a drug rash are other possibilities. A clinician may need to inspect the palms, soles, nails, and the rest of the skin.

Rinse away suspected irritants, use fragrance-free moisturiser frequently, and protect cracks with a simple dressing if rubbing is painful. Wear breathable footwear and keep sweaty areas dry. Avoid applying steroid, antifungal, or antibiotic creams at random because the wrong product can obscure the pattern or irritate broken skin. Note new medicines, products, foods, travel, and contact with anyone itchy; this history can be useful at review.

Seek prompt medical care if the rash spreads quickly, becomes hot or swollen, drains pus, or comes with fever or significant illness. Get urgent help for facial swelling, breathing trouble, faintness, widespread blistering, skin peeling, or sores in the mouth or eyes, particularly after starting a medicine. Pain that prevents walking or using the hands also warrants timely assessment.
