The iliotibial band is a firm band of tissue running along the outside of the thigh to the lower leg. Repeated knee flexion and extension can make pain appear at the outer knee, particularly after a change in distance, hills, speed or training surface. Weakness or poor control around the hip can increase strain on the movement pattern. Pain inside the joint, locking, giving way, marked swelling or pain after a twist points toward a different assessment pathway. Conditions such as knee bursitis, patellar tendinitis, meniscus tear or osteoarthritis may overlap in how they feel.

Temporarily reduce the activity that reproduces the pain, especially downhill running and repeated deep knee bends. Keep moving within a comfortable range, and use gradual warm-up before activity. A physiotherapist can assess hip strength, stride, footwear and training load, then build a progressive programme for the hip and knee. Avoid forcing a painful stretch or returning to full training while each step still provokes sharp outer-knee pain. Record the activity, location and timing of symptoms to make the assessment clearer.
When to seek care
Arrange medical review if pain follows a significant twist or impact, the knee becomes very swollen or hot, you cannot bear weight, or it repeatedly locks or gives way. Seek urgent care for severe pain with fever, a red hot joint, or calf swelling with breathlessness or chest pain. Persistent outer-knee pain despite reducing the trigger also deserves an examination.
