What swelling may indicate
Swelling is fluid and tissue response around the ankle. After a twist, an Ankle Sprain can cause tenderness, bruising, and difficulty walking; a fracture can cause more severe pain, deformity, or inability to bear weight. A hot, suddenly painful joint may occur with Gout, while gradual stiffness may occur with Ankle Osteoarthritis. These patterns overlap, so the appearance alone cannot establish the cause.

Rest from impact and keep walking within a tolerable level. Elevate the ankle when resting, use a wrapped cold pack for short periods, and choose supportive footwear. Do not massage a newly injured, very swollen ankle or apply heat while swelling is increasing. A clinician may assess joint tenderness, stability, circulation, and the need for imaging or guided rehabilitation.
When to get urgent help
Seek urgent care if the ankle is visibly misshapen, the foot is cold or numb, pain is severe after an injury, or you cannot take a few steps. Prompt review is also needed for rapidly increasing redness, warmth, fever, or swelling in one leg with breathlessness or chest pain. Do not delay assessment because swelling can obscure a significant injury.
