What should you know about painful and itchy rash?

A rash may be dry and scaly, raised, blistered, ring-shaped, or located in a skin fold. Atopic dermatitis often causes itchy inflamed patches, while contact dermatitis follows exposure to an irritant or allergen. Heat rash appears in blocked sweat areas. Intertrigo affects folds where moisture and friction collect. Ringworm can form a scaly ring, and scabies often causes intense itch with a household or close-contact pattern. Psoriasis may produce well-defined, scaly plaques. A drug rash can begin after a new medicine and needs prompt review.

These clues overlap, so the rash's appearance alone cannot confirm a diagnosis. Note when it began, where it started, new products or medicines, fever, and whether anyone close to you is itchy.

Wash with lukewarm water and a fragrance-free cleanser, pat dry, and avoid scratching, fragranced products, and tight clothing. Keep moist folds cool and dry, and do not share towels, bedding, or clothing when an infection such as ringworm or scabies is possible. Do not start a steroid cream on an undiagnosed ring-shaped or infected-looking rash without advice, because it can alter its appearance. A clinician can match treatment to the cause.

Get urgent help for a rash with breathing difficulty, facial or tongue swelling, faintness, blistering over large areas, skin peeling, mouth or eye sores, or severe illness. Seek prompt assessment for fever, rapidly spreading redness, marked pain, pus, or a rash that begins soon after a new medicine. Arrange a routine review when itch or pain persists, keeps returning, or disrupts sleep despite avoiding likely triggers.

Ask what features point toward eczema, contact dermatitis, infection, psoriasis, or a drug reaction. Bring a list of medicines, skin products, and recent exposures, and ask whether anyone close to you needs evaluation. Confirm whether the rash is contagious, which treatment to use, and what change should prompt reassessment.
