Shoulder pain when lifting your arm: what to know

Short answer
Pain when lifting the shoulder can involve irritated tendons, a stiff joint, neck-related nerves, or an injury that needs examination.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The short answer
Pain when lifting the shoulder can involve irritated tendons, a stiff joint, neck-related nerves, or an injury that needs examination. The height and direction that trigger pain, plus weakness, stiffness, or symptoms below the shoulder, help distinguish possible causes.
What lifting pain may mean
Lifting narrows and loads several moving structures around the shoulder. Rotator Cuff Tendinitis or Shoulder Impingement may cause pain through a particular part of the lift. A Rotator Cuff Tear can present with marked weakness, especially after an injury. Frozen Shoulder commonly limits both active and assisted movement, while Shoulder Osteoarthritis can bring stiffness and deep joint pain. Calcific Tendinitis may cause sudden intense pain. Neck irritation, including Cervical Radiculopathy, can send pain, tingling, or weakness toward the arm. These conditions overlap, so the movement alone cannot diagnose one.
What you can do
Avoid repeatedly lifting into the painful range and temporarily reduce overhead loads. Keep the shoulder gently moving within comfort so it does not become more guarded, but do not force a stretch or exercise that causes sharp pain. Support heavy objects close to the body and change position during desk or household tasks. A clinician or physiotherapist can assess neck and shoulder movement, strength, and function, then build a graded programme. Pain relief advice should take account of your health and other medicines.
When to seek care
Seek urgent assessment after a major injury, sudden loss of power, visible deformity, or a shoulder that cannot move. Get urgent help for chest pressure, breathlessness, sweating, or pain spreading to the jaw or arm. Prompt review is also needed with fever, a hot swollen shoulder, progressive numbness, or a hand that becomes cold or changes colour. Arrange an appointment if lifting remains difficult, pain disturbs sleep, or symptoms are not improving.
Questions for your doctor
Is the pain arising from my shoulder joint, tendons, or neck? Do I have true weakness or pain-limited movement? Which lifts should I avoid temporarily? Would physiotherapy or imaging help, and what should recovery involve?

Questions people ask
Can neck problems cause pain when lifting a shoulder?
Yes. Irritated neck nerves can produce shoulder or arm pain, tingling, altered sensation, or weakness during movement.
Should I stop moving a painful shoulder completely?
Usually avoid the painful load while maintaining gentle comfortable movement; complete rest may increase stiffness.
When is shoulder lifting pain urgent?
Urgent care is needed after major injury, sudden weakness, deformity, severe neurological symptoms, or chest-related warning symptoms.
