Pain Under the Nail of the Thumb: What to Know

A recent knock, trapped finger, manicure injury, tight glove, or repeated gripping can irritate the nail bed. Look for bruising, swelling, a split nail, tenderness at the nail edge, or a change in nail colour. Pain that is actually at the thumb base or worsens when the wrist and thumb move can reflect tendon irritation; De Quervain's Tenosynovitis is one condition a clinician may consider.

The pattern matters: throbbing after an injury differs from pain that builds with use, while warmth, drainage, or spreading redness can suggest inflammation or infection. Do not try to puncture the nail or remove it yourself.

Rest the thumb from forceful gripping and protect it from further knocks. Keep the nail clean and dry, use a cool wrapped pack briefly for swelling, and avoid tight coverings. If a ring or tight item is on the thumb, remove it early while swelling is limited. Note when the pain began, what movement or pressure triggers it, and whether the nail is changing.

Arrange prompt medical assessment after a crush injury, severe throbbing, marked swelling, a deep cut, loss of sensation, inability to move the thumb, or a nail that is lifting. Seek urgent help for rapidly spreading redness, pus, fever, or a thumb that becomes pale, blue, or cold. Persistent unexplained pain or a dark mark that does not grow out with the nail also deserves an examination.

Ask whether the nail bed, joint, tendon, or skin is causing the pain; whether imaging is needed after an injury; and how to protect the thumb while it heals. Mention any manicure, repetitive work, new swelling, colour change, or drainage.
