Pain Around the Thumbnail

A recent knock or trapped finger can cause tenderness beneath the nail, sometimes with bruising. Soreness at the nail edge with redness or drainage may involve the surrounding skin. If the pain is actually at the thumb base or wrist and worsens with gripping, the source may be a tendon rather than the nail; De Quervain's Tenosynovitis is one possible condition.

A new dark area, lifting nail, split nail, or change in shape should be described to a clinician, particularly when there was no clear injury. Nail appearance and the ability to bend the thumb help guide assessment.

Protect the thumb from pressure, picking, biting, and repeated gripping. Keep the nail clean and dry, and avoid cutting into a painful nail edge. A cool wrapped pack may ease soreness after a minor knock. Do not puncture or drain the nail yourself, because this can injure tissue and introduce infection.

If pain continues, a clinician can examine the nail, skin, joints, and tendon pathway. Mention recent manicures, chemical exposure, sports, or hand injuries.

Arrange prompt care for increasing redness, warmth, pus, fever, spreading pain, or a thumb that becomes difficult to move. After an injury, seek assessment for severe throbbing pain, a large blood collection under the nail, deformity, numbness, or loss of grip. New or persistent nail colour or shape changes without an injury also merit review.

Ask whether the pain is in the nail, surrounding skin, joint, or tendon; whether the nail needs treatment; how to protect it while it grows; and which signs would indicate infection or an underlying injury.
