What the movement can reveal

Pain during a particular part of elevation may occur when rotator cuff tendons become irritated or crowded beneath the shoulder roof, a pattern called shoulder impingement. Rotator cuff tendinitis can follow repeated reaching or lifting. Shoulder stiffness in every direction may occur with frozen shoulder, while deep aching and reduced movement may accompany shoulder osteoarthritis. Calcific tendinitis can cause an intense flare. Neck nerve irritation may add tingling or pain below the shoulder. These conditions can feel similar without an examination.

Steps that may help

For a short period, reduce the specific reaching, lifting, or sleeping position that triggers pain. Keep using the arm below the painful range for light tasks, and move gently rather than repeatedly testing the painful arc. Check workstation and sleeping support, and use cold or warmth according to comfort. Avoid heavy shoulder exercises until strength and movement have been assessed. A physiotherapist can tailor mobility and strengthening once the irritated structure is understood.

Arrange an assessment if pain is persistent, limits dressing or reaching, causes night disturbance, or follows a clear increase in activity. Seek urgent help after trauma with deformity, inability to use the arm, severe swelling, numbness, or a cold hand.
