Pain in the Groin and Testicle Area: What to Know

Pain that follows lifting, running, or twisting may involve the groin muscles or abdominal wall. Hip impingement, a labral tear, or hip osteoarthritis can refer discomfort toward the groin and testicle. Infection, a hernia, kidney or urinary problems, and conditions affecting the testicle or epididymis have different treatment needs. A clinician must examine the scrotum, abdomen, and hip rather than relying on location alone.

Avoid heavy lifting, sport, and movements that increase symptoms. Wear comfortable supportive underwear, and do not massage a swollen or very tender testicle. Note whether pain began suddenly, followed an injury, moves into the back, or occurs with urinary symptoms, fever, nausea, or a groin lump. Do not delay an examination if the testicle itself hurts or looks different.

Go to urgent care immediately for sudden or severe testicular pain, a high-riding or twisted-looking testicle, rapid swelling, nausea or vomiting, or pain after an injury. Seek prompt care for fever, burning urination, blood in urine, discharge, a painful groin bulge, or inability to pass urine. Emergency services are appropriate if pain is severe or you feel faint.

Ask whether the pain is coming from the testicle, epididymis, hernia, hip, or muscles; whether an ultrasound or urine test is needed; and which activity restrictions apply. Tell the clinician about onset, swelling, injury, sexual or urinary symptoms, fever, and pain that travels to the back.
