Forearm pain during biceps curls: what to know

Short answer
Forearm pain during biceps curls often reflects excessive gripping, wrist position, or load on the forearm muscles and tendons.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Technique focus
- Neutral wrist and a grip that is firm, not clenched
- Training rule
- Stop sharp pain instead of working through it
Why curls can hurt the forearm
A curl asks the biceps to bend the elbow while the forearm muscles stabilise the wrist and rotate the hand. A tight grip, bent wrist, fast lowering phase, or sudden increase in training demand can shift stress into those tissues. Inner-elbow or upper-inner forearm pain can occur with Golfer's Elbow; outer elbow pain can occur with Tennis Elbow. These patterns are possibilities, not a diagnosis. A pop, bruising, sudden loss of strength, or pain that travels into the hand needs assessment for an injury or nerve problem.
How to modify training
Stop the painful set rather than completing repetitions through sharp pain. For a return, lower the load, use slow controlled movement, and keep the wrist straight. A neutral-grip variation may feel easier than a fully turned-palm position, but do not use any variation that increases symptoms. Warm up with easy arm movements, avoid squeezing the handle harder than needed, and leave recovery time between demanding sessions. Do not test the area with repeated heavy curls while it is sore. A physiotherapist or sports clinician can check technique, strength, and the elbow and wrist together.
When to seek care
Arrange an assessment if pain returns with light exercise, persists between workouts, or limits daily use. Seek prompt care after a pop, fall, bruising, visible change, or sudden weakness. Urgent care is needed for severe swelling, loss of sensation, a cold or pale hand, or rapidly worsening pain.
Questions for your doctor
Ask whether the wrist position, elbow tendon, muscle, or nerve is driving the pain and which curl variation is suitable. Bring details of the grip, equipment, load changes, and the point in the repetition when pain starts.

Questions people ask
Can I train other muscles?
You may be able to train pain-free areas, but avoid exercises that reproduce forearm pain or require forceful gripping until assessed.
Why does changing the grip sometimes help?
A different grip changes forearm rotation and tendon loading, though relief does not prove a specific diagnosis.
