How lifting can trigger it

A muscle strain may begin during a heavy curl, pull, or sudden reach and can cause local soreness, tenderness, or pain with contraction. The biceps tendon connects the muscle to the shoulder and elbow, so irritation there may hurt with lifting, turning the palm upward, or reaching overhead. Shoulder movement can refer pain into the upper arm. Symptoms around the inner or outer elbow may also extend toward the forearm; golfers elbow and tennis elbow are examples of different elbow-side patterns.

A clinician may test shoulder and elbow movement, resisted strength, tenderness, neck and nerve signs, and the contour of the bicep. Sudden bruising or a change in muscle shape after injury needs prompt evaluation.

What you can do

Stop the lift that causes sharp pain and temporarily lower the load, range, or number of repetitions. Keep gentle shoulder and elbow movement within comfort so the arm does not become stiff. Cold wrapped in cloth may settle soreness after a recent overload; later, warmth can feel comfortable for some people. Do not repeatedly test strength with heavy curls. A physiotherapist can guide a graded programme for tendon or muscle capacity after the diagnosis is clearer.

When to seek medical care

Seek prompt care after a pop, sudden bruising, visible change in the bicep contour, marked weakness, or inability to bend the elbow or turn the palm upward. Urgent assessment is needed with chest pressure, breathlessness, or arm pain accompanied by feeling faint. Arrange review when symptoms persist, spread, or interfere with sleep or ordinary tasks.
