Understanding thumb pain

Thumb pain may follow repeated gripping, lifting, typing, phone use, or a direct injury. De Quervain's tenosynovitis affects tendons on the thumb side of the wrist and can hurt when turning the wrist, lifting a child, making a fist, or moving the thumb away from the hand. Joint irritation may feel deeper at the thumb base and make pinch tasks difficult. A sprain is more likely after the thumb is forced backward or sideways.

Notice whether pain is at the wrist side, the thumb base, or nearer the nail; whether there is swelling or bruising; and whether the thumb feels weak, catches, or becomes numb.

Comfort measures

Reduce forceful pinching, prolonged gripping, and repetitive thumb motions for a short period. Keep the wrist in a neutral, comfortable position and use the other hand for heavy tasks. A cold pack wrapped in fabric may help after a recent flare or injury. Do not force painful stretches or repeatedly test grip strength. A clinician or physiotherapist can check tendon gliding, joint stability, sensation, and hand function, then advise exercises or support.

When to seek care

Seek prompt care after an injury if the thumb is visibly crooked, very swollen, bruised, unstable, or difficult to move. Urgent assessment is also appropriate for a pale or cold thumb, new numbness, or severe pain. Arrange a routine review when symptoms persist, interfere with work or sleep, or return whenever gripping resumes. Early advice can prevent continued overload of an irritated tendon.
