Pain Before Your Water Breaks

Short answer
Pain before your water breaks can come from contractions, pressure, or another pregnancy concern, so timing and accompanying symptoms matter.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Track
- Timing, duration, location, fluid, bleeding, and baby movements
- Avoid
- Tampons or vaginal insertion when fluid leakage is possible
What the pain may mean
Labour contractions often build, last for a similar pattern, and become closer together. Pelvic pressure, backache, or period-like cramps can occur before fluid appears. Braxton Hicks tightenings may be irregular and may settle with rest or a change of activity. Pain with a gush or trickle of fluid may mean the membranes have ruptured, but fluid leakage can be difficult to distinguish from urine or normal discharge.
Before term, regular cramps, pelvic pressure, or backache can be a presentation of preterm labour. A maternity team may need to check your cervix, the baby’s wellbeing, and whether fluid is leaking.
What to do now
Note when each episode starts, how long it lasts, where it is felt, and whether movement changes it. Rest, drink fluids if you have not been told to restrict them, and use a clean maternity pad if you notice wetness. Do not insert anything into the vagina while you are assessing possible fluid leakage. Contact your maternity unit for advice if pain is recurring, worsening, or different from your usual pregnancy discomfort.
When to seek care
Seek urgent maternity assessment for a gush or continuous trickle of fluid, vaginal bleeding, severe or constant pain, fever, reduced baby movements, or regular tightening before your expected term. Go urgently as well for dizziness, fainting, or severe headache with visual symptoms. Do not wait for your waters to break if you feel something is wrong.
Questions for your doctor or midwife
Ask whether the pattern sounds like contractions, how to check for membrane rupture safely, and which maternity number to call overnight. If you are before term, ask what symptoms could signal preterm labour and whether you need an examination.
