Pain and spotting in early pregnancy

Light spotting can happen when the cervix is more sensitive, including after sex or an examination. Mild cramping may accompany normal changes as the womb adapts, but pain or bleeding can also occur with pregnancy complications, infection, or a pregnancy outside the womb. The amount of blood, pain location, timing, and whether a pregnancy test is positive all matter. Spotting does not by itself confirm that a pregnancy is continuing or ending.

Contact a maternity service, doctor, or early-pregnancy clinic for advice, particularly if the bleeding is new or pain continues. Record the colour and amount of blood, clots, pain location, and any dizziness or fever. Avoid inserting anything into the vagina until you have received advice if bleeding is active. Do not take medicines without checking that they are suitable in pregnancy. Assessment may include an examination, urine or blood testing, and ultrasound depending on the pregnancy stage and symptoms.

Go to urgent care for severe or one-sided pain, shoulder-tip pain, faintness, collapse, heavy bleeding, fever, or feeling very unwell. These symptoms can signal a time-sensitive problem and should not be judged by bleeding amount alone. If you have a positive pregnancy test with pain, seek same-day clinical advice even when the bleeding seems light.
