Pain and spotting during ovulation

Ovulation can cause a one-sided ache when an ovarian follicle stretches or releases an egg. A small change in reproductive hormones may also cause light spotting between periods. The discomfort may last briefly or come and go, and it should settle without becoming progressively worse. Bleeding between periods has other possible causes, including cervical irritation, infection, polyps, fibroids, or changes in the womb lining. A symptom alone cannot identify the cause.

Note the day of your cycle, which side hurts, how long the pain lasts, and the amount and colour of bleeding. Rest, gentle heat, and fluids may help. Use only pain relief that is suitable for you, especially if pregnancy is possible or you have stomach, kidney, bleeding, or medicine concerns. Arrange a routine appointment if this keeps returning, interrupts normal activities, occurs after sex, or comes with unusual discharge or fever. Your clinician may discuss pregnancy testing, infection testing, an examination, or imaging.

Seek urgent medical help for heavy bleeding, severe or worsening pelvic pain, faintness, marked dizziness, shoulder-tip pain, fever, vomiting, or a positive pregnancy test with pain or bleeding. These warning signs need prompt assessment because bleeding may not be related to ovulation. Ongoing bleeding should also be checked, including to assess the cervix and womb.
