Can ovarian torsion happen without a cyst?

A cyst or other ovarian enlargement can add weight, but it is not required. An ovary may twist because of the way it is supported, its position, or movement of the tube and nearby tissues. Torsion can also affect an ovary that looks unremarkable on an earlier scan.

The condition can reduce blood flow and cause sudden one-sided pelvic pain. Nausea and vomiting are common accompanying features, but symptoms can vary. Imaging may show an enlarged ovary, altered position or a twisted vascular pattern, yet no single scan result safely replaces clinical assessment.

Sudden pelvic pain with nausea or vomiting deserves urgent assessment, even if you have never had a cyst. Tell the clinician whether the pain is constant or comes in episodes, and mention pregnancy possibility, prior pelvic surgery and any fertility treatment.

Go to an emergency department for abrupt severe pelvic or lower-abdominal pain, repeated vomiting, faintness, fever or worsening tenderness. Do not dismiss the possibility of torsion because a previous scan did not show a cyst.
