What may be causing it

The outer calf can hurt after a muscle strain, a knock causing a muscle contusion, or repeated activity that overloads the calf and ankle. Pain may be worse when pushing off, stretching the calf, or walking uphill. Achilles tendinitis is usually felt nearer the back of the ankle and heel, but nearby pain can be confusing. Cramps, nerve irritation, joint problems, and circulation conditions can also cause calf symptoms. The timing, injury history, swelling, skin changes, and whether one or both legs are involved help guide assessment.

A clinician may check walking, ankle movement, calf strength, tenderness, pulses, sensation, and swelling. Do not diagnose a clot or muscle injury from location alone.

What you can do

After a recent minor knock or strain, protect the area from painful loading, use gentle movement, and gradually restore normal activity as comfort allows. Avoid forceful stretching or deep massage during an acute flare. Once serious causes have been excluded, rehabilitation can restore calf strength and ankle control; sports massage may be useful for selected muscular tightness, but it should not delay assessment of unexplained swelling or warmth. Hydration and a gradual activity build may reduce repeat overload.

When to seek care

Get prompt medical advice for one-sided calf swelling, warmth, redness, unexplained tenderness, or pain that began without an injury, especially after prolonged immobility or recent surgery. Call emergency services for chest pain, sudden breathlessness, coughing blood, faintness, or severe weakness. Seek urgent care after injury if the calf becomes very tense, pain is severe, or the foot is numb, pale, or cold. Persistent pain also deserves a clinical examination.
