Osteomyelitis of the Pubic Symphysis: What to Know

Short answer
Osteomyelitis of the pubic symphysis is an infection at the front pelvic joint that requires medical assessment and directed treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Location
- Front joint of the pelvis
- Common functional problem
- Painful walking or stair use
- Assessment
- Examination, blood tests, and imaging
What it means
The pubic symphysis joins the two halves of the pelvis at the front. Infection there may produce groin or lower abdominal pain, tenderness over the joint, pain when turning in bed, limping, or difficulty climbing stairs. Fever can occur but is not always present. Symptoms can resemble a muscle strain, pelvic joint inflammation, or a urinary problem, so examination and imaging are useful. Blood tests and, in selected cases, sampling of fluid or tissue help identify the cause.
What to do next
Seek a prompt clinical assessment, particularly when pain is worsening or walking is becoming difficult. Treatment may include prescribed antimicrobial medicine, activity modification, pain relief chosen for your health needs, and follow-up imaging or blood tests. Use walking support only as advised, because too much loading can aggravate pain while too little movement may cause stiffness. Do not self-treat with leftover antibiotics. If an abscess, unstable joint, or unhealthy tissue is found, a surgical team may discuss drainage or removal of infected tissue.
When to seek urgent care
Get urgent care for rapidly worsening pelvic pain, inability to stand or walk, fever with shaking chills, faintness, confusion, repeated vomiting, or new weakness or numbness in either leg. Emergency assessment is also appropriate for loss of bladder or bowel control, severe abdominal pain, or a wound with spreading redness or drainage.
Questions for your doctor
Could the pain be coming from the pubic symphysis rather than the hip or urinary tract? Which scan is appropriate? How much weight can I put through my legs? How will you check that treatment is controlling the infection? When should physiotherapy begin?

Questions people ask
Is pubic symphysis osteomyelitis the same as pelvic pain after exercise?
No. Infection can resemble an exercise injury, but persistent or progressive pain needs medical assessment rather than continued training.
Will walking make the infection spread?
Walking does not by itself determine spread, but painful loading may worsen symptoms; follow the clinician’s advice about weight-bearing and support.
