What should you know about osteomyelitis in pelvis?

The pelvis contains several connected bones and lies close to the hip, bowel, urinary tract, and reproductive organs. Infection may therefore feel like deep buttock, groin, hip, lower-back, or pelvic pain. Limping, reduced hip movement, night pain, fever, or feeling unwell can also occur.

Pelvic pain has many possible causes, including muscle, joint, urinary, and abdominal conditions. The presence of pain does not by itself show that bone is infected. A clinician needs to combine the history, examination, and investigations.

Seek prompt review for persistent deep pelvic pain, fever, a recent pelvic or abdominal infection, an operation, an injection, trauma, or unexplained difficulty walking. Tell the clinician about urinary or bowel symptoms, wounds, recent procedures, and medicines.

Assessment may include blood tests and imaging, often with attention to nearby organs and joints. Treatment can require targeted antibiotics and, when there is an abscess or damaged tissue, drainage or surgery. Rest and activity advice should be individualised.

Go to emergency care for severe or rapidly worsening pain with fever, confusion, faintness, inability to walk, new leg weakness or numbness, loss of bladder or bowel control, or a painful swollen area. These signs require immediate assessment.

Which pelvic bone or nearby structure may be involved? Could the symptoms come from the hip, spine, urinary tract, or abdomen instead? What imaging is needed? Are antibiotics, drainage, or specialist review part of the plan?
