Oral lichen planus under the tongue

The underside of the tongue is delicate and can be irritated by biting, dental edges, burns, infections, or inflammatory conditions such as oral lichen planus. Lichen planus may produce a lace-like white pattern, a tender red surface, or an ulcer. Appearance alone cannot confirm the cause. A dentist, doctor, or dermatologist may examine the whole mouth and neck, review medicines and triggers, and decide whether testing is appropriate.

Avoid spicy, acidic, salty, and very hot foods if they trigger pain. Rinse gently with water after meals, use a soft toothbrush, and avoid tobacco and alcohol-containing mouthwash. Do not scrape the area or apply leftover creams. If inflammation is confirmed, a clinician may recommend a corticosteroid designed for mouth tissue; more widespread symptoms may need another prescription approach. Take treatment exactly as directed and report new irritation.

Book an assessment for a patch, ulcer, or lump that does not settle, keeps returning, becomes harder, bleeds, or interferes with chewing, speaking, or swallowing. Seek urgent help for sudden tongue swelling, breathing difficulty, or inability to swallow fluids. A persistent under-tongue change deserves examination even when discomfort is mild.
