What should you know about oral antihistamines for hives?

Hives are raised, itchy patches or welts caused by histamine release in the skin. Individual welts often change shape or move, while new ones appear elsewhere. Triggers can include infections, medicines, foods, heat, pressure or may remain unclear. An oral antihistamine blocks histamine and may calm itching and swelling. Some older products make people sleepy and can affect alertness; do not drive or use machinery until you know how the medicine affects you.

Take only the recommended product and dose, and check with a pharmacist before combining cold remedies, allergy products or sedating medicines. Keep the skin cool, wear loose clothing and avoid scratching. Note possible triggers and the timing of each episode. If hives recur, last longer than expected, or interfere with sleep, a clinician can review the pattern and consider other treatment. Do not stop a prescribed medicine without advice, even if you suspect it triggered the rash.

Call emergency services immediately for swelling of the lips, tongue, mouth or throat, trouble breathing, wheezing, faintness, confusion or difficulty swallowing. Seek prompt medical advice for fever, bruising, painful lesions, skin peeling, sores in the mouth or eyes, or hives that began after a new medicine. Hives with airway symptoms can be part of a severe allergic reaction and should not be managed with an antihistamine alone.
