What should you know about open lumbar laminectomy?

Short answer
Open lumbar laminectomy is surgery through a back incision to remove part of a lumbar vertebra and relieve pressure on spinal nerves.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Surgical area
- The lower, or lumbar, spine
- Main purpose
- Create space for compressed spinal nerves
- Approach
- A conventional incision through the back
What the operation does
A laminectomy removes some of the lamina, the bony part at the back of a vertebra, so compressed nerves have more space. In the lumbar spine this may help symptoms such as leg pain, numbness, heaviness, or walking limitation caused by narrowing around the nerves. The surgeon may also remove thickened tissue or a disc fragment when needed. An open approach uses a conventional incision and muscle exposure rather than a small endoscopic access route. It does not automatically correct every source of back pain, and some people need an additional stabilising procedure if the spine is unstable. [Spinal Stenosis](/conditions/spinal-stenosis/) and [Spondylolisthesis](/conditions/spondylolisthesis/) are examples of conditions that may influence the plan.
Preparing and recovering
Before surgery, share your medicines, allergies, smoking status, medical conditions, and any bladder, bowel, or leg weakness symptoms. Plan help with transport, shopping, and bending tasks during the early recovery period. Afterward, follow wound instructions, walk as advised, and increase activity gradually rather than lifting or twisting before clearance. A physiotherapy plan may support posture, leg strength, and safe movement. Ask whether your case is suited to standard [Laminectomy](/treatments/spine-care/laminectomy/) or whether [Endoscopic Spine Surgery](/treatments/spine-care/endoscopic-spine-surgery/) is an option.
When to seek urgent care
Contact the surgical team urgently for new or worsening leg weakness, loss of bladder or bowel control, numbness around the groin, fever, increasing wound redness or drainage, severe headache after surgery, chest pain, or breathlessness. Sudden severe back or leg symptoms after a fall also need prompt assessment. Do not wait for a routine follow-up when nerve function changes.
Questions for your doctor
Which nerve is compressed, and what symptom is the operation intended to improve? Will decompression alone be enough? What restrictions, wound changes, driving guidance, and physiotherapy milestones should I expect?

Questions people ask
Is open lumbar laminectomy the same as spinal fusion?
No. Laminectomy removes bone to decompress nerves. Fusion is a separate stabilising procedure that may be considered when spinal movement or alignment is a concern.
What symptoms may improve after decompression?
Nerve-related leg pain, heaviness, numbness, or walking limitation may improve, while longstanding symptoms and mechanical back pain can behave differently.
