Onycholysis Toenail Causes: What to Know

Onycholysis means the nail plate has lifted away from the skin beneath it. The detached part may look white, yellow, or grey because air is passing under the nail. The appearance alone cannot identify the cause.

A single toenail that lifts after a knock, running, or tight footwear may reflect mechanical stress. Repeated pressure can separate the nail gradually, especially when the shoe presses on the front or side of the toe. Picking beneath the nail or trimming it too aggressively can extend the separation.

A fungal nail infection can change the nail's colour, thickness, and texture before or while it separates. Nail psoriasis can cause lifting, small pits, crumbling, or a yellow-brown patch. Inflammation around the nail, frequent wet work, irritant products, and certain medicines can also contribute. A clinician may examine the nail and, when infection is suspected, test a sample before choosing treatment.

Keep the toe clean and dry, and choose footwear with enough room for the nails. Trim only the loose edge with clean clippers; do not pull the nail off or probe underneath it. Cover a tender area with a light dressing that does not trap dampness, and change socks when they become wet.

Avoid nail polish, harsh solvents, and cosmetic nail work until the cause is clearer. Do not use leftover antifungal or steroid medicine without advice, because a lifting nail may have more than one cause. Arrange a routine dermatology or primary-care assessment if the change persists, spreads to other nails, or keeps returning.

Seek prompt medical care if the toe becomes increasingly painful, hot, swollen, or draining, or if redness moves into the surrounding skin. These changes can indicate infection or an injury that needs treatment. Prompt assessment is also sensible when a dark mark appears without a clear injury, the nail separates at its base, or several nails change unexpectedly.

People with diabetes, reduced circulation, nerve loss, or a weakened immune system should arrange medical advice early for a lifting toenail, even when discomfort is mild. Bring a list of medicines and mention recent trauma, footwear changes, sports, skin rashes, or exposure to prolonged moisture.

Ask what features point toward injury, fungal infection, psoriasis, or another cause, and whether the nail needs a laboratory sample. You can also ask how to trim the loose portion safely, which signs should trigger earlier review, and whether any medicine or skin condition could be contributing.
