One Thick Toenail: What to Know

Nail fungus commonly causes thickening with yellow, white, or brown colour, crumbling, or debris beneath the nail. A single nail can also thicken after repeated shoe pressure or an old injury. Nail psoriasis may cause pits, lifting, or colour change and can occur with skin or joint symptoms. Because these conditions overlap in appearance, a clinician may examine or test nail material before choosing treatment.

Keep the nail clean and dry, trim it carefully when softened, and choose footwear with enough room for the toes. Do not dig under the nail or share clippers. Photograph the nail and note pain, colour change, lifting, or nearby skin changes. Avoid starting oral antifungal medicine without a diagnosis and a review of your medicines and health history.

Arrange assessment if the nail is painful, lifting, draining, repeatedly catching, or changing despite gentle care. Seek prompt help for spreading redness, warmth, pus, severe swelling, or fever. A dark streak, pigment reaching the surrounding skin, or a new unexplained mark should be examined without delay. People with diabetes, poor circulation, or reduced sensation should seek advice early for foot or nail changes.

Ask whether the nail needs a fungal test, whether pressure or psoriasis is contributing, and what treatment duration and monitoring would involve. Discuss laser treatment for nail fungus and oral antifungal treatment only after the diagnosis and suitability are clear.
