Non-itchy hives: recognition and next steps

Hives, or urticaria, are usually smooth raised areas that may be pale, pink, or red and can appear on different body sites. Individual welts may fade while new ones appear elsewhere. Itching is common but not essential. Pressure, heat, infection, medicines, foods, or no clear trigger may be involved. Skin Rash and Skin Swelling can describe similar observations, while Angioedema refers to deeper swelling often affecting the lips, eyelids, hands, or feet. A photograph can capture a fleeting eruption for assessment.

Record when each episode starts, how long a welt remains, recent foods or medicines, infections, exercise, heat, pressure, and any swelling beyond the surface. Use cool compresses and avoid rubbing, tight clothing, hot showers, and known personal triggers. A clinician may discuss Antihistamine Treatment when appropriate. Do not stop a prescribed medicine without advice, and do not borrow someone else’s treatment.

Call emergency services immediately for swelling of the tongue or throat, trouble breathing or swallowing, wheezing, faintness, confusion, or rapidly worsening symptoms. Arrange prompt review if hives are widespread, painful, bruised-looking, persist in one spot, or recur frequently. A clinician may consider Oral Corticosteroid Treatment in selected situations, but this requires individual assessment.
