Non-blanching rash without fever

A non-blanching rash does not fade when pressed because blood may have moved beneath the skin rather than remaining in surface blood vessels. Small spots can look red, purple, or brown, and larger patches may resemble bruising. Causes range from minor irritation or pressure to inflammation, medication reactions, platelet or clotting problems, and infection. The rash itself does not identify the cause, and having no fever does not prove that it is harmless.

Check the rash in good light and take a photograph so you can compare changes. Note when it began, whether it is spreading, and any new medicines, illness, injury, or unusual pressure on the skin. Arrange same-day medical advice, particularly for a child, an older adult, or anyone taking medicine that affects clotting. Do not stop prescribed medicine without advice, but bring a medication list to the assessment.

Call emergency services immediately if the rash is spreading quickly or occurs with severe headache, stiff neck, confusion, fainting, breathing trouble, facial swelling, severe pain, repeated vomiting, unusual sleepiness, or a person who looks seriously unwell. Fever may be absent, especially early in an illness, so judge urgency by the whole picture.
