Sore throat without tonsils: what to know

Tonsil surgery removes the tonsils, not the whole throat. Viruses can still inflame the throat, including those causing a common cold, influenza, or COVID-19. Nasal drainage from allergic rhinitis may irritate the back of the throat, while acid reflux can cause burning or a recurring hoarse, sore feeling. Strep throat is also possible without tonsils because the bacteria can affect other throat tissue, although the pattern of symptoms and examination matter.

A sore throat may occur with fever, cough, runny nose, swollen glands, or difficulty swallowing. The combination and timing help a clinician decide whether testing or another explanation is needed.

Drink fluids, choose soft foods, and avoid smoke or other throat irritants. Warm drinks, cool foods, and gargling with appropriately prepared salty water may feel soothing. Follow label directions for any pain medicine and check suitability if you are pregnant, have a long-term condition, or take other medicines. Antibiotics do not treat viral sore throats and should only be used when prescribed.

Seek urgent care if you cannot swallow saliva, have trouble breathing, develop rapidly worsening swelling, or become unable to open your mouth normally. Arrange medical assessment for severe or persistent pain, a high fever, dehydration, or symptoms that improve and then return. After tonsil surgery, follow the specific discharge advice because bleeding requires urgent attention.

Ask whether your symptoms fit a viral illness, reflux, allergy, or possible strep throat; whether a throat test is useful; and which pain-relief options suit you. Tell them when your tonsils were removed and whether you have fever, cough, rash, or swallowing difficulty.
