Nicotine and Crohn's disease: what to know

Nicotine may alter bowel movement, appetite, heart rate and blood-vessel tone. Smoke exposure also introduces other substances that can irritate the body. Crohn's disease can involve any part of the digestive tract, so changes in abdominal pain or diarrhoea deserve attention even when they seem linked to nicotine use.

A flare can have several triggers, and a symptom after nicotine does not by itself prove that Crohn's disease is active. Treatment decisions depend on your symptoms, examination and medical history.

Tell your gastroenterology or primary-care team whether you smoke, vape, use nicotine pouches, or take nicotine replacement. Do not stop a prescribed Crohn's medicine because symptoms change. Keep a short record of nicotine timing, abdominal pain, stool changes, meals and medicines; this can help identify patterns.
