What should you know about nickel belt rash?

Nickel contact dermatitis often appears where a buckle or metal stud touches the skin, with itch, redness, scaling or small blisters. Sweat and rubbing can intensify the reaction. A belt-area rash can also be heat rash, intertrigo, atopic dermatitis, psoriasis, ringworm or scabies, so location alone cannot identify the cause. Contact dermatitis is a common presentation of nickel allergy, but a belt rash itself does not prove that allergy.

Remove the suspected buckle for now and keep the waist area cool, dry and gently cleansed. Wear a soft layer between the belt and skin, and avoid scratching. Photograph the rash before it changes and note whether its outline matches the buckle, whether it is scaly or moist, and whether anyone else has symptoms. A clinician can distinguish likely causes and may arrange patch testing. Treatment for inflammation can include a topical corticosteroid when prescribed.

Arrange an assessment if the rash spreads, persists after removing the belt, becomes painful, cracks, leaks fluid or repeatedly returns. Seek prompt care for warmth, pus, marked tenderness or fever, which can indicate infection. Get emergency help for breathing difficulty, facial swelling or rapidly widespread blistering.
