What should you know about neutropenic fever antibiotics?

Neutrophils are white blood cells that help respond to bacteria and other microbes. Chemotherapy can lower their number for a time, and fever may be the first visible sign of infection even when there is no obvious source. Clinicians assess temperature, symptoms, blood counts, organ function and possible infection sites. They may take blood cultures or other samples, but testing should not create unsafe delays in treatment.

The antibiotic choice depends on the person’s condition, recent medicines, allergies, local protocols and suspected source. Treatment may begin through a vein in hospital, with later adjustment when results and clinical progress are clearer. Do not use leftover antibiotics or wait for fever to settle on its own.

Use the thermometer and fever threshold provided by your oncology team, because instructions may differ between treatment plans. Call the emergency oncology number immediately when the team’s threshold is reached, or when you feel suddenly unwell during a period of low blood counts. Tell staff when chemotherapy was given, your current medicines and any antibiotic allergies. Growth factor support may be considered in some care plans to help the bone marrow produce neutrophils; it does not replace urgent assessment for fever.

Seek urgent medical care for fever or chills during a period when neutrophils may be low, even without cough, pain or another clear infection symptom. Also act promptly for confusion, dizziness, shortness of breath, chest pain, severe weakness, new rash, reduced urine, persistent vomiting or diarrhoea. Do not take a fever-lowering medicine to delay calling unless your oncology team has told you exactly how to use it.

Ask: When are my neutrophils expected to be lowest? What temperature or symptoms require an immediate call? Which emergency number should I use overnight? Where should I go for blood tests and antibiotics? Which medicines should I avoid? Could growth factor support be appropriate, and how will the next chemotherapy session be reviewed after a fever?
